The Albany Gazette - Germany's Digital Health Gap: Why Local Offices Are Unprepared for Future Pandemics

While the World Health Organization’s new pandemic early warning center in Berlin aims to detect any new disease outbreak within 24 hours by 2030, a stark disconnect exists between this ambition and the reality on the ground in Germany. The country’s roughly 400 local health offices are still struggling with the basics of digitalization, a gap that became glaringly apparent during the coronavirus pandemic when critical decisions had to be made based on data arriving via fax machines and manual spreadsheet entry.

Nicolai Savaskan, the Senior Medical Director and District Physician at the Berlin-Neukölln Health Office, and Mesut Yavuz, an independent IT consultant who led digitalization projects in Neukölln, highlight these systemic failures in a recent analysis. Their work underscores that technology alone cannot solve the deeper structural issues facing Germany’s public health service.

At the start of the pandemic, tracking incidence rates was the primary tool for assessing infection trends. However, authorities lacked the digital infrastructure to derive precise insights from raw case numbers. Modern statistical programs standard in academic settings could not integrate with the rigid administrative IT systems used by government offices. Savaskan recalled that their most advanced tools were limited to Microsoft Excel and a locally permitted Access database. This manual approach was prone to error; differing spellings of names, such as "Maier" versus "Meyer," often caused the system to count a single infected person as two separate cases.

It was only later in the pandemic that offices learned to better link key metrics with personnel capacities. Savaskan emphasized that digitalization must go beyond mere reporting to actively shape and ease on-the-ground work. With better tools, authorities could have identified vulnerable groups, such as residents in nursing homes or dialysis centers, for targeted protection rather than imposing blanket quarantines.

To address this backlog, the "Pact for the Public Health Service" was established with 800 million euros to promote open standards and reusable open-source solutions nationwide. However, Yavuz describes the current state of affairs as archaic, comparing the technology used in health offices to horse-drawn carriages in an era of electric vehicles. He noted that systems are not interoperable, preventing officials from sharing data across different platforms.

An analysis by Savaskan and Yavuz reveals that only five percent of these funds flowed into cross-state collaborative projects like "GA-Lotsen" in Frankfurt. The majority was used for local economic stimulus measures, as local politicians often preferred announcing high expenditures within their own states rather than adopting cheaper communal solutions. Furthermore, the financing for modern systems remains unstable. Federal support for the contact tracing software SORMAS ended in late 2022, and funding for the Robert Koch Institute’s (RKI) modern reporting system, EMIGA, is only secured until the end of 2026.

"If a pandemic broke out tomorrow, we would likely face very similar challenges," Yavuz stated soberly.

Beyond technology, the authors argue in their study "Political polarization and public health in Germany," published in Frontiers in Public Health alongside Alexandra Roth of the University of Lübeck, that political polarization has become an independent risk factor for public health. When medical recommendations are attributed to specific political camps in heated debates, public trust in institutions erodes, reducing the willingness of the population to support preventive measures.

Germany’s public health service is particularly vulnerable because it is directly integrated into the state administrative hierarchy and subject to political directives. Unlike clinics and practices, which were moved to self-governance after World War II to distance them from the state, health offices remained within the bureaucratic structure. This can create tension between medical expertise and political communication during crises. Savaskan pointed to the daily incidence reports issued by the RKI, noting that officials could feel when RKI President Lothar Wieler shared only partial agreement with the data for professional reasons.

To safeguard scientific integrity and public trust long-term, the authors call for fundamental structural reform. This includes legally protected professional independence, closer ties to science, a binding technical code of ethics, and eventually, self-governance for health offices as independent corporations. Savaskan clarified that political and democratic control is not contradictory to self-governance. Yavuz added that personnel, funding, and modern technology are insufficient on their own; people must be able to trust the decisions made by these institutions.
